Thanks for ordering from Skate Warehouse. Our online ordering system is very secure and ensures that no one can tap into your transmission and use your personal data. However, this does not stop people from entering false information.

This authorization form is a procedure to confirm that the authorized cardholder has made the order. This is an extra security measure we require to protect you, our customers and Skate Warehouse from fraud. We realize that this will take a little more time but the protection it offers outweighs the inconvenience.

The form below may be issued to new customers and previous customers using a shipping address and/or card number different to the one we have on file.

Required Information:
The following information will need to be emailed, or faxed before an order can be processed:

1. The name, address and telephone number of the bank issuing your credit card
2. A copy of your SIGNED credit card (both sides) *You are more than welcome to block out the first 12 digits of your card*
3. A copy of your personal identification card or driver's license (both sides)
4. The initialed & signed credit card authorization form below

*To copy this information you can use a scanner, fax machine or digital camera.

AUTHORIZATION FORM:

Please initial and complete the following:

government agency to determine any applicable duties and taxes
associated with my order.
____ I understand that I will be responsible to pay for all applicable duties and taxes upon delivery of my order.
____ I accept that if I refuse the delivery of my order, I authorize
Skate Warehouse to charge my credit card for the cost of the applicable
duties and taxes, as well as the return delivery charges back to Skate
Warehouse.
____ I understand that I will be responsible for any return shipping
costs associated with my order.
____ I understand if the package is abandoned, the possibility of being refunded for merchandise is void.

If you do not wish to provide the form, you are welcome to pay via international money order made payable to Skate Warehouse (in US funds), or through Paypal*.

*Please note:
-The name and email address of the Paypal account holder must match the billing information provided on your order
-The shipping information provided on your order must match the shipping name address provided with the Paypal payment

PLEASE READ THIS NOTICE:
There are no U.S. taxes; however, you will be responsible for any additional customs handling fees, duties and/or taxes. Skate Warehouse strongly suggests that you check with your local government agency to determine the extent of such charges prior to purchase.


If any international order is refused upon delivery for any reason, all customs & duty charges will be billed to you by Skate Warehouse. Once a package enters your country, these charges are not reversible whether the package is delivered to you or not.


Skate Warehouse will not issue credit in cases where packages are not delivered due to non-payment of fees, refused or abandoned.

(Please note: Orders that have already been shipped cannot be canceled.)
